Investigation Summary

Investigation Nr: 200070555
Event: 09/23/1998
Employee killed by truck trailer while crossing street

Employee #1 and a coworker had just arrived at work and were walking through one of the designated parking lots on their way to the building to punch in for work. They left the lot along the paved entrance and began to cross a T-intersection. Employee #1 stepped in front of a stationary cab-over-engine (COE) semi-truck, followed by the coworker a few feet behind him. The COE operator had stopped at the intersection and was waiting for the vehicular traffic to clear before making a left turn. He did not notice Employee #1 and the coworker, and struck them as he started to turn. Employee #1 was trapped between the two light mechanisms and could not get around the COE. He was crushed and killed by the rear tandems. The coworker was near the passenger side light mechanism and was able to push away from the moving truck. The designated parking lots, located at the back side of the company site, had only been used by employees for approximately two months.
