Investigation Summary
Employee #1 was operating a single-head notcher machine when the blade jammed. Employee #1 placed her hand inside the machine to remove a piece of wood which caused the jam. In doing so the machine became unjammed, and Employee #1's hand contacted the moving notcher blade, severing part of her left hand. Employee #1 was hospitalized.
