Employee killed in fall while threading range

Employee #1 was performing his normal job duties of threading range #1 when he fell 18 ft 8 in. from the range onto the concrete floor. He suffered traumatic head injuries and was killed. All the ladders had cages, all the work platforms had standard railings, and there was a standard railing across the top of the range.
