Employee injured in fall from ladder, died of heart attack

On January 3, 2001, Employee #1 climbed a fixed ladder to a roof hatch. After reaching the top of the ladder, he stepped down a rung or two, lost his grip and fell 12 to 14 feet to the concrete floor, breaking his pelvis. On January 12, 2001, Employee #1 was undergoing surgery to repair his pelvis when he suffered a heart attack and died.
