Employee killed in fall when platform collapses

On February 7, 2000, Employee #1 was working on a platform at a construction site when the collar slipped on the piling and the platform collapsed. He fell 20 ft to the ground and suffered fatal head injuries. The platform had been built after a snowstorm and during a period of the cold weather. The weather had then warmed substantially, but the supports had not been checked. The general superintendent later stated that the platform would support 60,000 lb, but it was determined that the platform had not been tested or adjusted to assure that it had the strength and integrity to support its working load. Employees had not received training on fall protection before the accident. Only after the accident did the employer hold a safety meeting on fall protection.
