Employee Struck By A Scaffold Part Falls 16 Feet And Dies

Employee #1 died when he was struck in the head by a scaffold part and fell 16 feet in a concrete pit. The crew Employee #1 was working with had just finished dismantling a section of scaffolding. While waiting for his coworker to climb down the scaffold ladder, Employee #1 sat or leaned on a guardrail when an unsecured diagonal scaffold part swung down and struck him in the head. The blow caused Employee #1 to flip over the guardrail and fall through a floor opening covered with plastic and land 16 feet below in a concrete pit. He died. The floor opening measured 12.5 feet by 25.5 feet. Employee #1 was a new employee and had only been working for his employer for approximately two weeks.
