One employee killed, one injured by falling conveyor

Employee #1 and a coworker, of Pro-Fab, and Employee #2 and a coworker, of the Northfield Company, were working under or near an I. J. White spiral conveyor machine being held up by four jacks. They were raising the spiral conveyor to replace a worn load bearing on the vertical shaft supporting the machine when the conveyor slipped off the jacks. Employee #1 was killed. Employee #2 sustained injuries for which he was treated, released, and then re-hospitalized. No cribbing or other means to secure the load from falling off the jacks was being used at the time of the accident.
