Employee #1 knocked off small step ladder
While putting cooling coils on for a hydrostatic test a gasket ruptured, and knocked Employee #1 off of a small two-step ladder and out of a shipping door. He landed about 20 feet away with a total drop of about 6 ft. The rupture was attributed to a failure to properly tighten down the test flange.
