Employee killed when struck by clamp truck

At approximately 12:35 p.m., on April 8, 1999, Employee #1 was standing on the floor at the west end of the winder deck, writing reject information on the face of a 76 in. diameter, 291 lb roll of paper, #A21591341. Approximately 18 1/2 ft away, a clamp truck operator began easing toward the winder deck, traveling at approximately 1 mph, with his clamps raised approximately 42 in. from the floor and positioned horizontally to remove roll a from the winder deck. As the clamp truck operator approached the winder deck, he struck Employee #1, pinning her between the clamps and the winder deck. She sustained internal crushing injuries and was transported to Jefferson Regional Medical Center, where she died at approximately 1:00 p.m. on April 9, 1999.
