Investigation Summary
On February 2, 1999, Employee #1 was working as a welder, approximately 15 feet from another employee. Employee #1 was working in the Assembly Area, Track II, repairing a stiffener, and grinding some porosity from a short jig car. When the grinder stopped, the employee collapsed, falling on his stomach with his head under the jig car. Employee #1 did not strike anything while falling 3 to 4 feet to the floor. The Medical Examiner reported that the cause of death for Employee #1 was Atherosclerotic Cardiovascular Disease.
