Employee died when struck head in pool

Two employees were closing an in ground swimming pool. In the middle of the 4-ft-deep section of the pool, is an island with a roof over it. Employee #1 obtained a ladder from a climbing wall to use to access a chimney that was located on the roof of the island. The ladder used was the upper section of a 24-ft-extension ladder. The coworker was standing on the island, under the roof, as the employee set up the ladder and climbed onto the roof of the island. The pitch of the roof was 12 in. 12 in. Employee #1 slipped on the roof, grabbing the ladder, and he fell 13 ft 2 in. striking his head on a stone and died as a result of blunt trauma to the head.
