Investigation Summary
At about 4:30 p.m. on September 2, 1999, Employees #1, #2, and #3 were seriously injured in an explosion that leveled the laboratory and destroyed several portable buildings. It was thought that the blast was initiated in the magazine which shared a wall with the laboratory. The exact cause of the explosion was undetermined due to the possibility of multiple ignition sources. Employees #1 and #2 were working inside the laboratory and Employee #3 was returning from his break and witnessed the explosion. Employee #1 died on September 7, 1999 from his injuries. Employee #2 received second-degree and third-degree burns to most of his body. Employee #3 that was not working in the lab broke his arm and was hospitalized and treated for some internal injuries. The accident was also inspected by the Bureau of Alcohol, Tobacco, and Firearms.
