Investigation Summary

Investigation Nr: 200962439
Event: 09/08/1998
Employee's eyes injured when sprayed by wet concrete

Employee #1 was operating a pour bucket that filled concrete plank mold trays with a cement mixture. Three 6 in. hard rubber hoses had been placed in the mold tray and pressurized to 50 to 60 psi so they would expand and produce hollow cores in the concrete planks. Two hoses became stuck together in the tray. Employee #1 used a 2 in. pipe as a spade to separate them. In the process, one of the hoses ruptured and the released air blew wet cement into his eyes. Employee #1 was hospitalized.
