Investigation Summary
Employee #1 was a member of a four-person crew directing trucks where to dump their loads on a highway widening project. The trucks had bi-directional alarms. The trucks would pull into a staging area, and back up approximately one mile to where the crew was stationed. Ticket takers, who were referred to as spotters, would position themselves between the barrier wall and the trucks. The spotters would direct the trucks to where they were to dump their loads, and then give them a receipt. Employee #1 had been instructed to sit in a company truck and wait for each truck. A dump truck was backing up with a load. The truck driver had to use his rearview mirrors to ensure that he did not drive into a culvert or run into a barrier wall which was on the operator's side of the truck. The driver was also looking for the spotter. Employee #1 apparently either heard or observed the dump truck approaching, and tried to get into his assigned position. Employee #1 tried to run behind the truck but was struck by the back wheels. Employee #2 was killed.
