Employee fatally burned from a ruptured tank

On August 21, 1998, Employee #1 was transferring hot driveway sealer at a temperature range of 375 to 600 degree Fahrenheit from Tank Number 24 to the pitch melter when the tank ruptured. The driveway sealer emanated from the tank on to Employee #1 burning 60 percent of his body. The supervisor had assigned him the job of transferring the sealant from the tank and previously helped him to shut off the valves at the pitch melter prior to the accident. While the supervisor was waiting for Employee #1 to meet him at the pump, another employee observed a cloud over Tank Number 24. Another employee located in the heat treater room felt a rumble, the employee got up to see what occurred. After walking out of the building, he saw Employee #1 was walking into the building. Employees assisted Employee #1 into the shower, and summoned emergency medical services. Later on, Employee #1 was taken to the hospital where he died from complications.
