Employee killed in fall with ladder

Employee #1 was installing a new residential television cable line. His ladder was leaning away from the residence and against the overlashed cable, which supported both the main distribution cable and the service line to the residence. The cable was attached to the house with a span clamp and a P-hook screwed into the outside wall. Employee #1 was working from the ladder when the P-hook pulled away from the house. The sudden release of tension in the cable caused Employee #1's ladder to whiplash and jerk backward. He was thrown 18 ft to the ground and was killed. Employee #1 should have released the tension in the cable before starting work.
