EQUIPMENT FALL ON EMPLOYEE BREAKING HIS LEG

At 1:00 p.m. on April 20, 1998, Employee #1 was jacking up a conveyor system on a portable rock crusher with a floor jack. Another employee placed the floor jack under the belted conveyor pulley. Employee #1 was partially under the conveyor system. The conveyor pulley turned allowing the floor jack to roll out from under the conveyor system. The conveyor fell on the lower part of the leg of Employee #1. Employee #1 was hospitalized with a fractured leg.
