EMPLOYEE KILLED DUE TO FALL FROM PLATFORM

An employee was performing maintenance on a steam hammer from a platform set on forks of a lift truck 10 ft from the ground. The working edges of the platform were guarded, but the platform was secured to only one fork. The employee moved to unsecured side of the platform and it slid off the fork. He fell to the ground and was killed.
