Investigation Summary
At approximately 6:10 p.m. on March 10, 1998, Employee #1 was working the #1 hot press machine, retrieving some loose panels from the output side of the hot press. The machine malfunctioned, ejecting a load of 30 sheets of plywood that struck Employee #1 in the head and chest. He was transported to LSU Medical Center, where he died at approximately 12:00 noon on March 24, 1998.
