Employee run over by front end loader is killed

At approximately 3:00 a.m. on July 6, 1998, three employees were involved with loading furnace slag into a dump truck. Two employees were scooping up the slag with front end loaders and dumping it into a dump truck. One employee was operating a rental front end loader and the other was operating a company owned Kobelco front end loader. The third employee, a laborer, was on foot keeping the slag in a neat pile. The employee operating the Kobelco front end loader dumped a load of slag into the dump truck and proceeded to back up, looking to his left rear but not to his right. In the meantime, a witness to the accident stated that Employee #1, a supervisor, was walking into the work zone with his head down. The witness lost sight of Employee #1 for a moment as the front end loader was backing up. A few seconds later he noticed that the loader had run over something and he motioned to the operator of the Kobelco front end loader to stop backing up. Employee #1 was on the ground and had been run over by the right front tire and was killed. The front end loader did not have a back up alarm, mirrors, seat belts and the front and rear lights were broken.
