Employee killed when crushed by falling load of pipes

Employee #1 and three coworkers were carrying a 576 lb bundle of 21 ft long pipes on their shoulders to place it on the 12 ft by 8 ft bed of a dump truck. The coworker carrying the front part of the pipe bundle set his end down on the bed of the truck, then turned to help push the load fully onto the bed. As he turned, the pipe slid off the truck and fell. Employee #1, who was shouldering part of the load, was knocked under the falling bundle of pipes. He died of crushing injuries to his head.
