Employee killed when struck by falling load of lumber

At approximately 2:10 p.m. on January 22, 1998, Employee #1, a carpenter, was working in the vicinity of a Pettibone forklift with a platform on the elevated forks. The platform was carrying two coworkers, thirty 4 by 8 sheets of 7/16 in. Oxboard stacked on the floor, and approximately ten 14 in. long 2 by 6 boards resting on top of the guardrails. Another coworker was lowering the platform when the forks tilted forward, causing about 21 sheets of the Oxboard and half of the 2 by 6 boards to slide forward and out of the platform. The unsecured platform also slid forward, but did not come off the forks. As the sheets of Oxboard moved forward, they hit a wall and then fell, striking Employee #1 on the side of his head. Emergency services was called and paramedics arrived on the scene within minutes. Employee #1 was transported to the regional hospital, where he was pronounced dead on arrival due to closed head trauma.
