EMPLOYEE DIES AFTER FALL FROM LADDER

On October 24, 1997, Employee #1, of Holbrook Waterproofing Company, was using a roof hatch fixed ladder in an existing building to access a roof. Employee #1 lost contact with the ladder and fell approximately 20 feet to a stairwell. The employee sustained cranial injuries and died from his injuries on November 27, 1997.
