Employee's arm partially amputated in grinder

At approximately 6:00 p.m. on April 24, 1997, Employee #1, a 20-year-old material handler for R. A. Serafini, Inc., a plastic injection mold manufacturer, was told by his supervisor to clean out an IMS granulator/grinder before the molds and color were changed for a new production run. Employee #1 apparently did not ensure that the main power source for the grinder was unplugged. He opened the top of the hopper and used a wet/dry vacuum to clean out the grinder. While doing so, he stuck his left arm through the cutter blades and was cleaning out the area below them when the machine activated and cut through his lower arm, slicing both bones. Employee #1 got out of the machine and was taken to a local hospital, where his arm was reattached in emergency surgery. He was hospitalized until April 28, 1997.
