On November 18, 1998, Employee #1 was assigned to hold an aluminum extension ladder for coworkers who were engaged in repair work on a roof. He decided to climb to where they were working when the ladder slipped out and he fell between 30 and 35 ft to the ground. Employee #1 was killed.
