Employee injured in fall off platform

At approximately 7:15 a.m. on November 12, 1998, Employee #1 was on a work platform on a Skytrack lift truck. He was walking to the end of the platform when it slid off the forks. Employee #1 fell approximately 8 to 9 ft, amputating his lower leg. The forks of the lift were not placed through the receiver boxes on the platform.
