Investigation Summary
On October 3, 1997, Employee #1 was operating an orderpicker. Employee #1 fell from the orderpicker and was found unconscious. The forks of the orderpicker were 9 feet, 6.5 inches from the ground and were still located in the air after Employee #1's fall. Employee #1 died January 6, 2000 as a result of the injuries he sustained from the accident on October 3, 1997.
