Employee injured when trench wall collapses

Employee #1 was at a construction site, helping to install an 18 in. plastic drain pipe in a 7 ft 6 in. deep trench that was 5 ft wide at the top. A Caterpillar 320 BL trackhoe was used to dig the excavation and the pipe was lowered and set in place using a chain attached to the trackhoe's bucket. Employee #1 got into the trench and straddled the pipe. He was unhooking the chain when one side of the trench, composed of slate rock, shifted and collapsed, pinning his leg and hip against the pipe. Employee #1 suffered a sprained ankle, degloving of the skin on the back of his right leg, and separation of his right tibia and fibula. Emergency services was called, and Employee #1 was transported to the hospital. The trench had been dug in a clay-type soil that had sections of slate rock. The terrain was wet and previous rains had left the trench and the surrounding soil.
