Investigation Summary
On June 27, 1997, the B shift was working from 7:00 a.m. to 7:00 p.m. in the cloth room when at about 2:30 p.m., several cloth graders began experiencing headaches. The employer's management began an investigation of the cause of the complaints, calling in the local fire department, who monitored for carbon monoxide. A large inventory of cloth was in the room and two propane-powered fork trucks were operating almost continuously. Employees #1 through #9 were sent to the hospital, where blood samples registered carboxyhemoglobin levels of 3.4, 6.6, 11.7, 6.5, 16.2, 11.1, 7.6, 10.4 and 7.5, respectively, indicating an overexposure to carbon monoxide. At 5:30 p.m., levels of carbon monoxide were recorded at 10 and 13 ppm. A leading dock door was raised and fans brought in to increase ventilation.
