Employee killed when crushed by steel beams

At approximately 10:30 a.m. on February 19, 1997, Employee #1 was unloading steel beams from a delivery truck and stacking them outside in the saw area. He used a 7 1/2 ton hoist to pick up a 30 ft long, 6,210 lb extra-heavy-duty steel beam and set it down on top of a pile of steel beams. This 6 to 7 ft tall pile consisted of beams stacked six vertically and overlapped inside-out. Employee #1 set the 30 ft long beam down on top of this stack, unhooked the metal webbed cradle chain from the hoist and began to walk away. At that moment the stack of six beams shifted and fell, with three of the beams landing on Employee #1. He was trapped and died within five minutes of multiple injuries.
