Employee killed in fall from ladder while changing light

While attempting to change an overhead light bulb, Employee #1 leaned too far to one side, tipping the nine-foot aluminum ladder over. Employee #1 fell, striking his head on the side of the swimming pool, and landing half in the pool and half out. Employee #1 died from his injuries.
