ONE EMPLOYEE INJURED AND ONE KILLED WHEN PALLETS FELL OVER

On December 13, 1996, a forklift operator was operating a forklift in an area where plastic parts containers were stored. The area, identified as the Container Storage Area, was located next to employees working at floor level and next to aisle where assembly operations were being performed. These automotive parts containers measured approximately 4 feet by 4 feet and were one foot thick. These particular containers weighed approximately 165 pounds each. The forklift bumped or caused a stack of plastic parts containers to slide, which in turn fell over onto two employees who had no physical protection from being struck by falling containers. The result was one employee being injured and a second employee being fatally injured due to heart damage.
