Investigation Summary
On May 21, 1999, Employee #1 was spraying fireproofing material when he began to feel sick. He told his coworkers that he did not feel well and that he was going to go outside for some fresh air. About 15 minutes later he was discovered in his car with no pulse. Emergency personnel could not revive him and he was taken to the hospital where he died. The autopsy stated that Employee #1 died as a result of fluid in the lungs due to a pulmonary infarction of an undetermined cause.
