Investigation Summary
Employee #1 was working under a forklift, Model Number V160, Serial Number 16V359, weighing 2,598 lbs with a 160,000 lbs capacity. One of the two-ton jack stands supporting the forklift snapped in half causing the forklift to fall on him. The right step of the forklift crushed his neck and pelvis and he was killed.
