Employee died in trench cave-in

On October 9, 2001, an employee of Island Roads, Inc. was preparing piping for installation. The process momentarily took the employee outside the protection of a trench box that had been provided. There was a collapse of the adjacent facing of the unprotected trench walls where the employee had been at the time of the accident, trapping and pinning the employee approximately two feet from the trench box. The employee died of asphyxia.
