Employee Amputates Finger Tip in Unguarded Pusher

At approximately 2:40 a.m. on October 7, 2005, Employee #1 was working as a temporary employee for LA Corr Packaging, Inc., a manufacturer of corrugated sheets. He was employed with the company for approximately two months. As they were working, a coworker tried to clear a line of corrugated card board boxes that were backed up on a conveyor, by pushing a button that would start the pusher. As the pusher was moving, Employee #1 picked up some cardboard sheets that were left behind and attempted to place them on top of the cleared stack. Employee #1's right index finger got caught between the pusher, which was unguarded, and the conveyor, smashing his right index finger and amputating his finger tip. He was transported to Kaiser Permanente Medical Center, where he was treated for his injuries.
