Employee Is Burned in Fire Welding a Fuel Tank, Dies Later

At approximately 6:15 p.m. on October 31, 2005, Employee #1, of Sukut Construction Inc., was doing welding work to fix the leak on a Caterpillar fuel tank, model Number 651 B, at a construction site. There was still some diesel fuel left in the storage tank, with a capacity of 200 gallons, which was not emptied and flushed prior to welding. There was no testing done to determine the vapor concentrations and lower explosive limit of fuel vapors inside tank. At about 6:30 p.m., a fire erupted during the welding work due to ignition of fuel in tank. The fire quickly spread in the work area and ignited Employee #1's coverall, resulting in third-degree burns to his lower torso. He was transported to Foothill Presbyterian Hospital by paramedics and later transferred to Sherman Oaks Burn Center in Sherman Oaks, where he died on November 15, 2005 due to burn injuries.
