Investigation Summary

Investigation Nr: 201800067
Event: 06/28/2005
Employee Amputates Left Middle Finger

At approximately 9:45 a.m. on June 28, 2005, an employee working as a machine operator, observed a cap that fell off the end of a tube that was on the hopper of a corn-curler machine. An auger was in the tube, transporting corn meal from the hopper to the machine. The auger was rubbing against the cap during many hours of operation, which caused the cap to wear out become crooked. As the employee was placing the cap on the tube that housed the auger, his finger was pulled in to the tube due to the movement of the auger. Two-thirds of employee's left middle finger was amputated.
