Investigation Summary

Investigation Nr: 201799988
Event: 06/07/2005
Employee's Leg Is Crushed When Run over by Truck

At approximately 1:30 a.m. on June 7, 2005, Employee #1 was working as a hostler at a freight transportation and warehouse business. A driver of a 48 ft freight truck drove his vehicle from the mid-west during the night. The driver pulled inside past the security gate at the facility and parked in the yard and went to a nearby room for a coffee break. Illumination was adequate in the yard. Employee #1 started to disconnect the air lines from the truck to detach the trailer. He went under and near the 5-th wheel of the truck to pull the pin. The driver came back and started to move the truck without insuring no one was in the vicinity. Employee #1's right leg and foot were crushed, and he was hospitalized.
