Employee's finger is amputated in truck lift gate

While Employee #1 was making a delivery to a customer, he opened the lift gate of his truck. The lift gate opened abruptly and the employee's left ring finger became caught. The tip of the left ring finger was amputated. Further investigation revealed that there was a sticker on the side of the truck that instructed employees to stay clear of the side of the lift gate while opening it. The employee had been a truck driver with the company for 7 years and was experienced in the operation of the lift gate.
