Employee's Toe Is Amputated by Forklift

At approximately 10:00 p.m. on November 23, 2004, Employee #1 stepped off the pallet jack he had been driving to pick up some items from a rack on aisle B. As he stepped down, another worker was driving a forklift in the opposite direction and ran over Employee #1's right foot, amputating his little toe. The aisle was approximately 2.5 inches less than the necessary width for two pallet jacks to pass. Also, per the worker, he was not looking in the direction of travel when the pallet jack he was driving struck Employee #1.
