Employees toes crushed between pallet jack and metal post

At approximately 8:45 a.m. on July 14, 2004, an employee was stocking office supplies in the warehouse of Office Depot. He was a pallet jack operator, Crown Model Number PE 3000, who was receiving cross-training on how to pull products from the cage area. Products are stored on racks in this area. He was being trained on where different products were located and the shelf numbering system. He placed products on the forks of the pallet jack and was riding on the platform. He was making a 180-degree turn from aisle E to go into aisle F. The pallet jack made a wide turn and the platform hit a metal post of the cage. Two toes on his right foot were crushed between the pallet jack platform and the metal post and were amputated. He was hospitalized due to his injuries.
