Employee's Foot Is Fractured in Fall from Ladder

At approximately 4:00 p.m. on June 9, 2004, an employee, a communication-installation technician, was at a newly constructed building to install the wires for the communication system. The warehouse in the building was partitioned with a wall that was 12 ft 9 in. high. An extension ladder was placed against the wall and the employee ascended the ladder and installed wires on the platform that was on top of the wall. As he was descending, the ladder slipped out and fell on the floor of the warehouse, causing the employee to fall onto the floor. The employee's right foot was fractured. He was hospitalized and treated for the fractured foot. The ladder was not tied or blocked to prevent it from displacing at the time of the accident. The extension ladder was 32 foot long and manufactured by Louisville.
