Glass Panel Falls and Injures Employee

At approximately 4:30 a.m. on March 5, 2004, Employee #1 and a coworker were manually handling glass panels using a portable rack. A stack of approximately 20 glass panels, 63-in. long by 42-in. wide by 0.25-in. thick, weighing approximately 400 pounds, fell on Employee #1 from the portable rack. The load knocked Employee #1 down resulting in a fracture to his right leg. The portable rack was standing still at that time and its wheels were choked by wood blocks. The load fell sideways a distance of approximately 2-ft to 4-ft feet and knocked Employee #1 down. Employee #1 suffered a fractured right leg and sprained left knee and was hospitalized for six days.
