Investigation Summary
At 11:10 a.m. on July 18, 2002, Employee #1, a facilities worker at California Polytechnic Institute in Pomona, was using a dolly to move a metal desk across the threshold of a door in the gymnasium. He moved the front wheels across the threshold but when he pushed on the desk to move the back wheels over it, the back wheels collapsed. The load fell backward onto Employee #1, fracturing his ankle. He was hospitalized for his injuries.
