Employee Riding on Forks of Lift Truck Is Injured

At approximately 10:15 a.m. on October 19, 2004, Employee #1, a plant maintenance man, was on the roof of the cold storage locker. He was fixing water leaks and called a forklift operator to give him a ride, a stand up type electric forklift. He climbed onto the forks and was elevated. While ridding he reached out to the back guard of the truck and his fingers were caught in the closing forklifts mechanism. Employee #1 received cuts to his finger.
