Investigation Summary
At 8:00 a.m. on December 10, 2003, Employee #1 was operating an Ameripak PFM 2500 model number 1420 bread wrapping machine at the Country Village Baking facility in Montebello, CA. He amputated the distal phalanx of the third and fourth fingers on his right hand. Employee #1 was taken by his supervisor to the Immediate Medical Center of Commerce facility in Paramount, CA. His middle finger was reattached and his fourth finger was amputated down to the first knuckle. He was hospitalized. Bread passes through this machine to be wrapped. There are no extension tools provided or offered by the employer for use with this machine. It is equipped with a swing-down hood that has a plastic guard door at the entrance of the knife's point of operation. There is a 2.125 in. gap from the plastic guard door to the edge of the blade. This door guard is supposed to prevent an operator's fingers from touching the knife blade, but Employee #1 stated that the door must remain open for the bread to fit through. Employee #1 has been operating the machine since 1994, and he was hired by Country Village Baking when they purchased the machine in 1997. He stated that he had never been trained by Country Village Baking to operate the machine, though they did train him on bakery ovens and mixers.
