Employee's Leg Is Fractured by Falling Slitter Head

At approximately 7:00 a.m. on February 15, 2004, Employee #1, a maintenance mechanic, was operating a forklift, Yale walk behind lift, Model Number MCW030LAN24CS083, Serial Number N499290. He was hoisting a load when the slitter head slipped off and fell on his right leg. He received a cut to his right thigh, a broken right knee, and a sprained left ankle. Employee #1 was hospitalized.
