Employees Are Injured When Struck by Wheel

At approximately 12:00 a.m. on June 10, 2004, Employee #1 was working as a CNC operator. He was operating the Mori Seiki lathe, Model Number: SL-45M. He was running aluminum wheels on the lathe. He ran a part and the lathe started to vibrate. Employee #2 was also working as a CNC operator. He was in charge of setting up lathes. He was told by the supervisor to assist other operators during the shift. Employee #1 asked Employee #2 for help on the lathe he was operating, saying that the wheel was wobbling. When the program finished on the lathe, they reran the program and heard vibrating. They reloaded the same wheel, closed the door, and ran the cycle again. The wheel weighed 120 to 130 lbs. The wheel came off the machine. Employee #2 pushed Employee #1 out of the way. The wheel hit Employee #2's left elbow and left foot. The wheel then hit Employee #1's left hand. Emergency medical services were called, and the employees were taken to the Medical Center of Santa Ana. Employee #1 had multiple fractures on his left hand and was in the hospital for five days. Employee #2 had a laceration on his left elbow that required sutures, and he had multiple fractures on his left foot. Employee #2 was not hospitalized
