Investigation Summary

Investigation Nr: 201691110
Event: 09/29/2003
Worker Amputates Thumb While Using Metal Polishing Machine

At approximately 3:20 p.m. on September 29, 2003, Employee #1 was working as a leadman in the polishing department at Al's Plating, in Los Angeles, CA. He was operating a Junior Bluff Co. polisher, with no reported model or serial number, for the firm, a traditional electroplating manufacturer where finish coatings are applied to door hardware. The polisher had a 1,400-rpm motor with dual shafts. Employee #1 had installed a spacer on one of the shafts so that he could install a second wheel and polish parts faster. He was sanding down the spacer area of the polisher while it was running at full speed. He sized up the spacer twice, by turning off the machine, and sized up the spacer to the shaft again. When it did not fit, he then turned the polisher back on to sand down the spacer some more. As he was sanding, the sanding belt pulled both his arms into polisher machine's shaft area. Employee #1 was wearing a glove, when the shaft point or arbor end amputated part of his left thumb. No one witnessed the accident. Emergency medical technicians took him to Community Hospital in Gardena. The employer was cited for a serious accident-related violation under Section 4237(a).
